To assist in making your office visit as simple as possible, we have provided an outline of some things that you can do to help make your check-in as smooth as possible: 

• Our forms are loaded on our website, which we would ask you to print off and complete prior to your appointment.   Please be sure to fill them out in their entirety and bring them with you.  You can present them to our Patient Care Representative upon check in at the front desk.
  
  
• Eye Care Associates is proud to participate with most insurance companies. To aid in the timely billing of your visit, please be sure to bring your current insurance cards, so that we may copy them for accurate information.
  
  
• If your insurance company requires that you have a referral for your visit, please be sure to contact your Primary Care Provider prior to your visit to obtain such referral. 
  
  
• Also, please be aware that any co-pay that you may have is due to at the time of your visit.  We conveniently accept checks, Mastercard, Visa and Discover.

• Lastly, if the patient that we are seeing is a minor, a parent or guardian must accompany them to provide permission to treat the child.  If a parent or guardian is not present, it is our policy to reschedule the appointment.
